Based on what manufacturers have been showcasing recently, are we actually getting real performance enhancements in pistols or are they just giving us some minor cosmetic updates?

The only one I know about is the Glock Gen-6 vs Gen-5.
Mostly comfort/ergonomic and cosmetic changes with a couple function upgrades:

"Revamped grip with palm swells and smart contours
 Gas-pedal thumb rests cut into frame
 Undercut trigger guard
 Flat-faced trigger with shorter reach
 New optics mounting system
 Improved checkering
 Large beavertail
 Modified extractor
 Single-stage recoil spring assembly"

Takes Gen 5 mags
Reversible mag catch
Comes with three magazines
Includes two extra grip backstraps

Great analysis on the Gen 6 changes! You’re spot on that most of it focuses on ergonomic and cosmetic tweaks rather than performance improvements. The upgrade in optics mounting is probably the standout functional enhancement. This trend seems to be the norm in the industry these days. A lot of the "new" pistols are just small enhancements. Genuine performance breakthroughs are pretty rare since modern striker-fired pistols already function quite well. Manufacturers are more about refining existing features than reinventing the wheel. The real upgrades usually come in the form of better optic mounting systems, enhanced factory triggers, and ergonomic adjustments that accommodate a wider range of hand sizes. It might not be fun, but these changes definitely enhance usability for many shooters.
